Participant Waiver

Blue Note Karaoke — Liability Waiver & Release

By signing this waiver, I acknowledge that I am voluntarily participating in a Blue Note Karaoke event organized by Blue Note Karaoke, LLC ("Company"), located at 10922 Schuetz Road, Saint Louis, MO 63146.

1. Assumption of Risk. I understand that participation in karaoke activities involves certain risks, including but not limited to physical exertion, use of audio equipment, and interaction with other participants. I voluntarily assume all such risks.

2. Release of Liability. I hereby release, waive, discharge, and covenant not to sue Blue Note Karaoke, LLC, its owners, employees, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by me during participation in any Blue Note Karaoke event.

3. Photo/Video Release. I grant Blue Note Karaoke, LLC permission to photograph and/or video record my participation and to use such images or recordings for promotional and marketing purposes, including social media, without compensation.

4. Medical Conditions. I represent that I am in good physical health and have no medical conditions that would prevent my safe participation. I agree to inform Blue Note Karaoke staff of any relevant health concerns prior to the event.

5. Conduct. I agree to conduct myself in a respectful and appropriate manner during all Blue Note Karaoke events. I understand that disruptive or inappropriate behavior may result in removal from the event without refund.

6. Governing Law. This waiver shall be governed by the laws of the State of Missouri.

This waiver is intended to be as broad and inclusive as permitted by Missouri law. If any portion is held invalid, the remainder continues in full force.
